Wherever possible, this should be used as the basis for compiling a report of a spontaneous phenomenon investigation

Key Sheet Template

Unless clients have given specific permission for publicity, investigation reports should contain pseudonyms for clients and witnesses.   A key sheet should be completed to cross-reference pseudonyms with real names and addresses.
